If you're Pre-diabetic or you just want to change your health;  sign up to participate in the Diabetes Prevention Program. 

Follow these steps:

• Download the referral form, complete it, email, mail or fax it back to us ASAP. 
• Speak with your doctor before participating in this program or any lifestyle change program. 
• After your referral application has been accepted; monitor your email for instructions on when  classes will start.
• Commit yourself to the program and continue to   live your best life yet.
